Thematic review article related to the hospital discharge or discharge plan from nursing care. The search was made from different studies conducted in Colombia, in the period 2009 - 2015, for which literature linked to the subject was reviewed, which served as a support to discuss, reflect and critically evaluate the data obtained in the research advanced, with respect to the perspective that other authors have. 23 articles were analyzed, divided into three groups, related to self-care agency, education and nursing process, all associated with the discharge plan. It is concluded that, since the discharge plan is an activity that should be included in any health institution, this activity should be led by the nursing staff.
